Medical Report Request

Medical reports are documents requested by patients for insurance claims (pay and claim), courts, employers, EPF, SOCSO or police reports. Patients’ written consent is compulsory for the release of the medical information. For patients below the age of 18, the parent or legally acceptable representative is required. If the patient is deceased or incompetent, consent is required from the next of kin of the patient or the legally acceptable representative appointed by the family or family members of the patient.
